Why aftercare matters
Early recovery brings new routines, social dynamics, and stressors. Consistent support reduces relapse risk and strengthens confidence. Our approach combines structured follow up, peer connection, and skills refreshers so you keep moving forward.
What is included
Personalized relapse prevention plan
You will leave with a written plan that identifies triggers, warning signs, coping strategies, and emergency steps. We revisit this plan at scheduled check ins.
Alumni groups and community events
Regular alumni meetings, skill tune ups, and sober activities create connection and accountability. You will meet peers who understand your journey and celebrate milestones with you.
Ongoing therapy options
Individual, group, and family sessions are available as needed. Your team can coordinate with community providers for long term support.
Recovery coaching and life skills
Coaching covers routines, sleep, stress, budgeting, job readiness, and communication. We help you practice what works and adjust what does not.
Housing and resource navigation
When appropriate, we connect clients with safe housing options and local resources that support stability.
Medication and psychiatric follow up
For clients using Medication-Assisted Treatment or receiving Psychiatric Evaluation and Medication Management, we schedule ongoing visits and coordinate refills.

When to reach out
Contact us if you notice rising stress, cravings, mood changes, sleep disruption, or shifts in your support system. Early outreach prevents small problems from becoming crises. Alumni can request rapid skill sessions or brief clinical check ins to get back on track.
